How to Register

To register with our practice you need to live within our practice boundary area – enquire with reception if you’re unsure.

Once you have confirmed you live within our boundary area please follow the link below to complete the registration form.

After you have completed and submitted the forms you must e-mail us 2 forms of identification (ID) to the surgery within 7 days. If your ID documents are not received within 7 days, your registration will be CANCELLED. E-mail address to send ID documentation:

Acceptable Identification;

  1. Proof of who you are; driving licence, passport or birth certificate AND
  2. Proof of your current address; tenancy agreement, mortgage agreement, bank statement or utility bill
    Until you have submitted your ID you will not be registered at the practice.

The registration process can take up to 14 days. In the meantime, please seek medical advice from your previous GP practice.
